What this means
Double-underscore strong emphasis (__text__) must be balanced
inside the same block. This error fires when an opening __
appears without a matching close before the block ends.
Why this happens
Most often a typo, or a literal __ pair (e.g. __init__) read
as an emphasis opener.

Add the closing __, or escape the underscores when they belong
